We are seeking a highly skilled Triage Registered Nurse (RN) to join our Patient Call Center team and provide care and support. As a Patient Call Center (PCC) RN, you will be responsible for assessing patient needs over the phone and providing appropriate medical advice and care. You will work closely with physicians, nurses, and other healthcare professionals to ensure patients receive timely and accurate support. In this role, you will be critical to ensuring patients' medical needs are addressed efficiently and effectively. In addition, you will be cross-trained as a Clinical Support RN to provide support as needed. Upon successful completion of orientation and training, this position offers the option to work remotely one day per week. Hourly Range: $33.50 - $41.00, based on years of relevant experience.

Requirements

  • Graduate of an accredited school of nursing
  • Current Registered Nurse (RN) licensure in the State of MN required.
  • Current CPR Certification
  • Minimum of 1 year experience
  • Excellent communication and interpersonal skills
  • Ability to perform a variety of nursing care tasks

Responsibilities

  • Collect information regarding patient’s condition via telephone- including acutely ill patients, and ensure needs are met using TPB and/or licensed practitioners orders
  • Document all activities, interventions, patient/family responses, medication dispensed or prescribed, hospital data, and other relevant information in the medical record
  • Answer multiple phone calls for all clinic locations in a timely, polite, and professional manner
  • Efficiently use the EMR system to document patient records
  • Per TPB and/or Voyage Healthcare Provider Approved Standing Orders: 1. Explain next steps to patients (e.g., speak with provider, schedule same-day appointment)
  • Coordinate care by scheduling appointments based on the urgency of the call
  • Promote wellness by providing patient education, communicating provider instructions, and following clinic-specific protocols
  • Provide information regarding diagnostic procedures, medications, nutrition, and general health maintenance per the primary care provider recommendations
  • Relay information from other professional callers, including pharmacies, long-term care facilities, home care, and hospitals
  • Send written messages to clinic providers requesting medical advice, prescriptions, or follow-up
  • Complete accurate, clear, and thorough documentation of all telephone encounters
  • Maintain confidentiality regarding all patient, provider, and employee information
  • Complete call-backs for messages, lab results, and diagnostic imaging
  • Prepare letters and orders for the providers to sign
  • Scans letters and forms into the EMR
  • Comply with procedures and policies to ensure a safe work environment and maintain OSHA compliance
  • Performs other related duties as assigned
