About The Position

The RN - Triage Team provides immediate clinical support within the PACE center by prioritizing participant needs, managing urgent concerns, and coordinating care. This role ensures timely response to participant and staff inquiries, supports safe medication administration, and facilitates smooth clinic operations. InnovAge is dedicated to empowering seniors to live independently, allowing them to age in their own homes and communities safely. InnovAge offers an alternative to nursing homes through its Program of All-inclusive Care for the Elderly (PACE), which provides enrolled seniors with customized healthcare and social support at PACE Adult Day Health Centers. Requirements

  • Current RN license in state of practice.
  • Current First Aid and BLS certification (American Heart Association or Red Cross).
  • 1+ year experience with frail or elderly populations.

Nice To Haves

  • BSN degree.
  • 1+ year acute care experience.
  • 3+ years healthcare experience with geriatrics emphasis.

Responsibilities

  • Perform rapid assessments and determine care priority; escalate urgent cases to providers.
  • Manage participant and phone calls, routing appropriately based on clinical urgency.
  • Perform procedures (IVs, phlebotomy/venipuncture, point of care testing, wound care, injectable medications) as prescribed/ordered.
  • Coordinate same-day appointments and urgent referrals.
  • Administer medications and treatments as prescribed; monitor compliance and educate participants.
  • Maintain accurate documentation in participant medical records.
  • Support interdisciplinary team communication and participate in care planning meetings.
  • Ensure triage area is stocked and organized for efficient workflow.
  • Provide health education and counseling to participants and caregivers.
  • Comply with all regulatory and InnovAge policy standards.
