About The Position

Empath Health is currently seeking a RN - Triage/Field to join our team. 8pm to 8am, 36 hours a week, flexible hours The RN - Triage/Field: Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team (in in the patient home, long term care facility, assisted living facility, hospice house or hospital) Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care. Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care. Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions. Reviews and updates the Plan of Care, physician orders, MAR/Pharmacy profile and other records to ensure patient/family needs are met.

Requirements

  • Current, unrestricted Florida Licensure as RN.
  • Minimum one year recent acute care experience in a Hospital or Home Health environment; previous experience as a Hospice RN preferred.
  • An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
  • BSN or MSN degree preferred.

Nice To Haves

  • CHPN certification is strongly encouraged within 3 years of employment.
