About The Position
Empath Health is currently seeking a RN - Triage/Field to join our team. This position involves visiting patients and providing clinical, physical, educational, and emotional support after-hours as directed by the Triage team. The RN will work in various settings including patient homes, long-term care facilities, assisted living facilities, hospice houses, or hospitals. The role requires performing patient assessments, managing discomfort, and providing symptom relief using specialized nursing skills related to palliative and end-of-life care. Additionally, the RN will utilize nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for the patient and their family unit. Responsibilities also include attending patient deaths, providing bereavement support to caregivers and families, and performing post-mortem care. Accurate and timely documentation of patient services within electronic medical records is essential to reflect the individualized plan of care and outcomes of interventions. The RN will also review and update the Plan of Care, physician orders, MAR/Pharmacy profile, and other records to ensure patient and family needs are met.
