About The Position

The RN Transfer Center Coordinator at PeaceHealth is responsible for managing and coordinating all aspects of incoming direct admissions and patient transfers at a 24-hour operation transfer center. This role acts as a liaison for referring entities and ensures efficient patient flow and throughput by facilitating communications between transferring and accepting physicians, coordinating inter- and intra-hospital transfers, and maintaining bed management for the Network.

Requirements

  • Accredited School of Nursing required; Bachelor's Degree in Nursing preferred.
  • Minimum of 2 years clinical nursing experience in an acute care hospital required; previous leadership experience preferred.
  • Registered Nurse credential required; Basic Life Support certification required upon hire.
  • Excellent critical thinking and problem-solving skills.
  • Demonstrated superior service excellence skills.
  • Proficient computer skills including Microsoft Office and CareConnect applications.
  • Excellent multitasking, prioritization, organizational, and interpersonal skills.
  • Strong verbal and written communication skills.
  • Ability to function as a team player and work with diverse individuals.
  • Basic knowledge of statistics and reports.

Responsibilities

  • Coordinates multiple processes to connect referring entities with appropriate service requests and facilitates care coordination.
  • Initiates protocols for inter-facility transfers, including STEMI, Stroke, and Trauma protocols.
  • Facilitates and coordinates all inter- and intra-hospital transfers, ED admissions, and direct admits.
  • Coordinates communications between transferring and accepting physicians and other healthcare providers.
  • Utilizes multi-line telephones and integrated transfer center software for conference calls with providers.
  • Documents and maintains all transfer information and communications.
  • Facilitates inpatient bed management using integrated bed management software.
  • Assesses facility capacities and capabilities in collaboration with House Supervisors and Management.
  • Maintains all-physician call schedules for the Network.
  • Ensures compliance with state and federal laws/regulations affecting practice.
  • Performs proactive reviews of medical documentation for utilization review purposes.
  • Facilitates communications regarding in-house expirations and coordinates with ancillary facilities.
