About The Position

This position is Part Time (20 hours weekly). Typically works Monday through Friday schedule. The Telehealth Baby Step Program RN interacts on-site face-to-face with physicians, staff, and healthcare professionals in planning, providing, promoting, and supporting families being discharged from the Neonatal Intensive Care Unit (NICU). Establishes strong in-person connection with families at the bedside to introduce the Baby Steps program, build rapports, and ensure understanding of the program's structure and goals - this face-to-face engagement is a critical element for program success. Develops and fosters communication and relationships to promote access to Baby Steps services, which continue through virtual care. Responsible for scheduling, registration and coordination of Telehealth consultations to support the infant's transition from NICU to the home environment.

Requirements

  • Bachelor's Degree in Nursing
  • RN Licensure within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) - maintain active and in good standing throughout employment
  • American Heart Association BLS - maintain active and in good standing throughout employment
  • Neonatal Resuscitation Program (NRP) required within 1 year of hire - maintain active and in good standing throughout employment
  • 2-4 years of NICU RN experience
  • Bilingual in English and Spanish

  • Pediatric nursing in an Acute Care setting preferred.

Responsibilities

  • Contacts new and existing patients via phone/e-mail to communicate TeleHealth consult requirements.
  • Meets families in-person at the bedside.
  • Serves as a liaison between patients, families, physicians, and interdisciplinary teams.
  • Ensures connectivity success and requirements completion before each TeleHealth call.
  • Completes post-TeleHealth call patient documentation.
  • Assists with TeleHealth training.
  • Supports in call center customer support including information and technical troubleshooting calls. Escalates technical issues to Web Development Team as needed for resolution.
  • Identifies and eliminates health care system barriers to ensure timely access to quality medical and clinical care.
  • Identifies, informs & educates patients about their disease. Provides educational materials to parents. Develops care plans to enhance education & satisfaction; addresses clinical concerns.
  • Serves as a nurse liaison to facilitate services on patient management and leads in the coordination of complex care across a continuum of care from admission and procedure through outpatient follow-up.
  • Ensures clinical protocols to refer patients to appropriate specialist for diagnosis and treatment. Assists patients/families in obtaining referrals/appointments with needed specialists.
  • Coordinates with NICU Nurse Navigator Rounds: RN is expected to participate in NICU complex care rounds in order to understand clinical care and progress of NICU infant, anticipate upcoming discharge, and develop relationships with clinical team caring for infant.
  • Responsible for documenting the encounter in real time in the clinical record. Additionally, collects and tracks patient- and encounter-specific information required for evaluation and metrics.
  • Helps to disseminate Baby Steps work including but not limited to: conference attendance and presentation, manuscript development, grant development, webinars, podcasts.
