About The Position

Your job is more than a job The RN Supervisor - Transfer Center is responsible for coordination and communication of transfer requests for LCMC Health Adult Hospital Market (AHM) facilities by using problem-solving, critical thinking, and excellent customer service skills. This professional will act as a facilitator and decision-maker while applying nursing knowledge as a foundation for placement decisions and communications. This role will also be responsible for bed planning and throughput facilitation at designated LCMC Health Adult Hospitals. Participates in the day-to-day operations of the department, under supervision of nursing manager/director. Acts as a clinical and operations resource person maintaining current knowledge of clinical practice and departmental operations. Audits, reports, and educates staff on nursing documentation and meeting professional and JCAHO requirements. Prepares the staffing schedule with approval of the appropriate manager/director. Plans daily staffing coverage according to department needs and appropriate skill mix. Adheres to the staffing plan and monitors staffing level on a daily basis. Modifies staffing patterns based on patient acuity, current technology, and available resources. Supervises departmental employees to include conducting new employee orientation and competency assessment. Provides feedback and input for performance evaluations and handles personnel matters as delegated/designated by department manager/director. Works in collaboration with the leader(s) in interviewing and selecting candidates for open positions. Attends recruitment events and represents the department when requested. Assigns preceptors to new orientees and provides orientees with orientation materials. Reviews orientation packet for completion by the preceptor and sends completed paperwork to the leader. Maintains employee accountability with all hospital policies and procedures. Encourages participation from employees in the ongoing delivery of care and/or services. Your Everyday Serve as the “point person” to facilitate incoming transfers/transports, maintain communication with all parties including but not limited to attending/accepting MD and referring facility. Uses problem-solving skills to determine best course of action which will be satisfactory to the patient and referral health care team and optimize communication with the appropriate physician. Utilizes nursing experience and education as foundations for gathering accurate clinical data to assure appropriate and prompt transfer and placement of the patient. Responsible for documenting, tracking, and coordinating logistics for hospital transfers and bed planning. Establishes and maintains a high degree of awareness and knowledge of hospital’s medical staff, services and programs. Maintains professional and technical knowledge through continuous education and accepts new assignments and performs additional duties as requested.

Requirements

  • 3 years experience in inpatient nursing / call center / critical care / ED
  • American Heart Association Basic Life Support/CPR
  • Licensed Registered Nurse through the Louisiana State Board of Nursing
  • Basic Nursing skills with high level critical thinking ability.
  • Exhibits excellent customer relationship skills including telephone and personal communication with all customer groups (physicians, administrators, nurses, case managers, and other team members).
  • Proficient in hospital computer systems for accurate data collection, monitoring and evaluation.
