Givens Aldersgate is looking for an RN Supervisor who will assist the Executive Director of Aldersgate at-Home in managing the day-to-day clinical operations. In this role, you will be responsible for assessing, implementing, evaluating, and documenting client care. You will supervise, validate competency, and provide ongoing education to team members involved in client care.

Requirements

  • Valid RN license.
  • 1 + year of nursing experience

Responsibilities

  • Coordinate the services the client is requesting while also facilitating the client’s efforts towards self-sufficiency and independence.
  • Ensure agency is following state regulations by assessing and evaluating client status by reviewing and implementing plan of care, regularly evaluating client’s needs and revising the plan of care as necessary.
  • Effectively communicate the plan of care to the in-home aid and client and/or responsible party.
  • Plans for client care based upon assessment, along with the medical plan of treatment.
  • Supervises team members and delegates responsibilities when required.
  • Communicate with the client, physician, family members and other members of the care team.
  • Teaches client and family/ caregiver self-care techniques as appropriate.
  • Complete documentation and other record keeping.
