Job Title: RN Supervisor, Home Health Job Location: Dunmore, PA / Lackawanna and Luzerne County Shift: Full-time Job Duties / Responsibilities include but are not limited to the following: Demonstrate exceptional professionalism and care coordination via thorough communication with team members, patients and family / caregivers, physicians, referral sources, payer sources Direct daily office clinical activities including patient scheduling and triaging calls from physicians, employees, and patients Review clinical records and documentation to verify regulatory and agency compliance including infection control standards, evaluation of infection control logs, identify and address trends related to patient infections Identify and discuss operational issues, keeping staff informed of additions or changes in policies Participates in performance improvement activities and provides backup support to staff Maintains current practice knowledge through internal and external training programs / seminars and acts as a resource for the interpretation of clinical practice / standards Assist with recruitment, interviewing, selection, and orientation process of new team members Participate in community events such as health fairs, conventions, and sales calls to provide clinical expertise Evaluate performance related to job goals and requirements, provide feedback Participate in special projects and performs other duties as assigned Why Choose Traditional Home Health? Requirements

Current license as a Registered Nurse in Pennsylvania
Home Health experience is A MUST
Excellent assessment and documentation skills and ability to meet deadlines
Strong interpersonal and leadership skills with a positive, pro-active attitude
Ability to work independently as well as collaborate with team members
Excellent verbal / written communication, time management, and organizational skills
Must have a current drivers license
Ability to travel to case assignments PRN
