The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.
Responsibilities
- Provide safe, holistic care to patients with multiple co-morbidities and advanced chronic illnesses.
- Educate patients and families regarding self-management.
- Support the needs of patients, families, and the community.
- Collaborate with team members to promote quality, compassionate, holistic, patient-centered care.
Requirements
- Minimum of 1 year med-surg experience required.
- 1 year of home care experience preferred.
- BSN preferred.
- Able to work weekend, holiday, and on-call hours.
- IV and venipuncture experience preferred.
- Able to demonstrate professional competencies in a home care setting.
- Able to efficiently utilize technology (cell phone, computer).
- Able to travel in service area.
