About The Position

AHRC NYC is looking for a Part-Time Registered Nurse (RN) for their Midwood Respite residence in Brooklyn. Experience working with indivuals with Dual Diagnosis (IDD and Mental Health) is preferred. The RN will evaluate and monitor health and safety needs of individuals with developmental disabilities to promote wellness. The RN ensures that delivery of nursing care is conducted in compliance with all federal and state regulations, as well as agency policy. In urgent/emergent situations, the nurse may receive phone calls outside of routine working hours regarding people residing in the five-bed emergency respite/hospital diversion program. Pay Rate: $48-$50 per hour depending on experience. Schedule: 14 hours per week....flexible schedule. Location: 1460 East 13th Street in Brooklyn

Requirements

  • Must be currently licensed as a registered professional nurse (RN) in New York State.
  • CPR certification required.
  • Must demonstrate excellent written and oral communication skills.
  • Ability to handle multiple priorities and work as a member of a team.

Nice To Haves

  • Experience working with people with dual diagnosis (IDD & Mental Health) preferred.
  • Valid Tri-state area Driver's License and good driving record preferred.

Responsibilities

  • Works in collaboration with the FCS Clinical team to screen people referred to the emergency respite house within 48 hours of screening by FCS Clinical team.
  • Collaborates with FCS Clinical team to coordinate medical care for assigned individuals in the emergency respite/hospital diversion program, monitor and assess health needs including arranging for follow-up care as needed, supporting identified emotional/psychiatric and social needs in the aftermath of crisis/emergency incidents.
  • Assessment by direct observation when appropriate, of people’s health status. Observe for signs and symptoms of discomfort, distress or illnesses. Evaluation of physical and psychiatric status changes for exacerbation of existing conditions, onset of disease, and/or responses to care and treatment. Documentation and follow up of all findings.
  • Communicate with hospitals, physicians, families, FCS team members and clinicians involved in providing services to individuals.
  • Communicate and follow up on hospitalizations, emergency room visits, reports of injury, trauma, accidents, illness, distress and other health issues as individually required.
  • Attend meetings to discuss any health-related issues regarding people supported as needed.
  • Supervises AMAP administration of prescribed medication and routine treatments in accordance with federal and state regulations and agency policy. Participates in in-services and trains AMAPs as needed. Completed AMAP certification, recertification and decertification processes as necessary.
  • Regularly reviews medication administration records (MARs), treatment and adaptive equipment records, logs documenting vital signs, elimination, glucose readings, weight, menses, and other records as individually specified. Reports significant issues to appropriate clinicians and team members, including need for ordering medications, renewing or updating prescription orders and individual specific drug information sheets. Monitor and manage medication storage and disposal procedures in accordance with federal and state law.
  • Monitor people’s dietary and nutritional needs. In-service and support staff in managing special meal and feeding of individuals served.
  • Participate in discharge planning including writing recommendations for referrals to other agencies and follow up care.
  • Ensure that referrals are made for routine and special medical services, and follows up afterwards to ensure that recommendations and orders are appropriately carried out and managed.
  • In compliance with OSHA mandates, monitors infection control, environmental changes and ensures that all guidelines regarding universal precautions are appropriately followed. Order and maintain necessary first aid and medical supplies as needed.
  • Review Incident and Medication Error Reports. Provide appropriate follow up action as necessary.
