Responsibilities: Responsible for collaboration, communication & facilitation of optimal provision of care in addition to being involved in the provision of direct care of patients and families. Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge. Provide quality holistic nursing interventions, med/surge nursing interventions and hands on skills including class II telemetry monitoring and chemotherapy administration, respiratory support, critical thinking and more. Collaborative team approach: SNAP rounding daily with interdisciplinary team including physicians, clinical affiliates, medical social work, care managers, nutrition, Rehab services, spiritual care, and pharmacy. Ensures quality nursing care is rendered to all patients in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards of care and practice. Ensure a seamless, effective, efficient transition of care across the continuum. Participates in monitoring quality metrics specific to the department & ensure complete & accurate documentation in the patient record. Act as 1) mentor; 2) clinician; 3) educator; 4) consultant/collaborator; and 5) navigator.
