Responsibilities

  • Perform duties to meet the physical, psycho-social, and spiritual needs of rehabilitation patients.
  • Gather necessary data in a timely manner and monitor patient conditions.
  • Incorporate cultural and ethnic factors into patient care approaches.
  • Assess patients using rehabilitation nursing policies and procedures.
  • Demonstrate a rehabilitation knowledge base in duties performed.
  • Develop, implement, and evaluate nursing aspects of the trans-disciplinary care plan.
  • Initiate discharge planning on admission and coordinate patient care across the continuum.
  • Administer pharmacotherapeutics safely and competently.
  • Ensure patient safety based on the physical environment and equipment.
  • Provide care in emergency situations and follow established procedures.
  • Communicate patient care/service issues through the organizational chain of command.
  • Identify therapeutic rehabilitation patient care issues and determine appropriate interventions.
  • Respect and maintain patient rights, ensuring privacy and dignity.
  • Determine the presence and type of Advance Directive and involve patients/families in decision-making.
  • Identify patient/family educational needs and provide individualized teaching.
  • Demonstrate knowledge of age-specific care and communicate effectively.
  • Demonstrate effective Team Leader/Change Nurse skills.
  • Practice Universal Precautions.

Requirements

  • Graduate of an RN Training Program.
  • 1 year of Registered Nursing experience in an acute care setting.
  • Current California RN license.
  • BLS card.
  • Fire Card.
  • Assault Response Competency (ARC) required within 30 days of hire.

Nice-to-haves

  • Recent experience in Rehab Care.
