About The Position
KeystoneCare is looking for a dedicated and compassionate RN Registered Nurse to join our Home Health team on a full-time basis. The ideal candidate will deliver high-quality, patient-centered care in the home environment, ensuring the best possible outcomes for our patients. In this role, you will be responsible for assessing patient needs, developing personalized care plans, and providing direct nursing care that meets those needs while adhering to our established policies and procedures. You will also work collaboratively with physicians, interdisciplinary teams, and families to ensure seamless coordination of services and promote patient independence. Requirements
- Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
- Graduate of an accredited nursing school (BSN preferred).
- At least 1 year of nursing experience; experience in home health preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and valid Driver's License.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
- Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
Responsibilities
- Conduct comprehensive patient assessments to evaluate their healthcare needs.
- Develop, implement, and update individualized care plans based on patient assessments and preferences.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient progress and responses to treatment.
- Educate patients and their families on health management, medication administration, and care plans.
- Coordinate care with interdisciplinary team members to promote optimal health outcomes.
- Ensure compliance with agency policies, state, and federal regulations.
- Act as a patient advocate, promoting their rights and preferences in care.
