About The Position

Midland Care Connection is a not for profit, community based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Hospice House A peaceful, 12 bed, in patient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team—nurses, social workers, chaplains, and other specialists—focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life. RN Weekend Team Leader Provides leadership and supervision for weekend and on-call hospice staff, ensuring high-quality patient care and compliance with agency policies. The role combines clinical expertise, administrative oversight, and direct patient care. Responsibilities include guiding and training staff, monitoring documentation and quality standards, coordinating admissions and patient visits, participating in on-call rotation, serving as a pharmacy liaison, and supporting policy and procedure development. The team lead also engages in community education, quality improvement initiatives, and staff development, ensuring weekend operations run smoothly and patients and families receive consistent, compassionate hospice care

Requirements

  • Current, unrestricted Kansas RN license
  • Strong interpersonal and communication skills
  • Critical thinking and decision-making skills

Nice To Haves

  • Experience in hospice or palliative care
  • Previous leadership or supervisory experience
  • Experience with EMR systems and clinical documentation
  • Knowledge of hospice regulations and standards of practice
  • Experience in quality improvement initiatives and staff training
