This position is for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) specializing in pediatric home care. The role offers the opportunity to provide skilled, one-on-one nursing care to patients in their homes, fostering consistent schedules and meaningful relationships with patients and their families, free from the typical chaos of facility environments. Requirements

• Active IL RN or LPN license (or Compact license)
• Strong assessment and communication skills
• Compassionate, dependable, and self-motivated
• Valid driver’s license and reliable transportation

Responsibilities

• Provide skilled, one-on-one nursing care in the home
• Administer and manage medications
• Monitor and document patient conditions
• Perform treatments as ordered by the physician
• Educate and support family members
• Collaborate closely with the Phoenix clinical team
