About The Position

This full-time Registered Nurse (RN) Patient Navigator position is within the Women's Health Administration department at UAB St. Vincent's Birmingham. The role involves serving as a patient navigator throughout the continuum of care, functioning as a liaison for various stakeholders, coordinating treatment efforts, promoting quality improvement, compiling data, tracking outcomes, and providing referrals/support for uninsured or underinsured patients. Requirements

  • Licensed Registered Nurse credentialed from the Alabama Board of Nursing obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing and Required professional licensure at time of hire.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Responsibilities

  • Serve as a patient navigator throughout the continuum of care.
  • Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Coordinate efforts for treatment and promote quality improvement and educational efforts.
  • Compile data, track outcomes, and make recommendations for process improvement.
  • Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
