About The Position

Join our dedicated team at MercyOne Waterloo Medical Center, where RNs have a unique opportunity to deliver patient care through virtual platforms. In this role, you will assess and care for patients using video conferencing technology, ensuring seamless admissions, discharges, and patient education within our acute care unit.

Requirements

• Current Iowa RN license.
• BSN preferred.
• Minimum five (5) years of equivalent experience in health condition management in an adult acute care clinical setting with direct patient care experience within the last 10 years.

Responsibilities

• Collaborate, communicate & facilitate optimal provision of care through a computerized &/or digital platform supporting daily management of care.
• Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.
• Utilize clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care.
• Ensure a seamless, effective, efficient transition of care across the continuum.
• Monitor quality metrics specific to the department & ensure complete & accurate documentation in the patient record.
• Perform the roles: mentor, clinician, educator, consultant/collaborator, and navigator.
