About The Position

In the Midland Care Connections Community Health hospice program, an RN Case Manager plays a critical role in coordinating and delivering high quality, patient centered end of life care. They are responsible for assessing patients’ medical needs, developing individualized care plans, and ensuring that patients receive appropriate symptom management, medications, and therapies to maintain comfort and dignity. The RN Case Manager also serves as the primary clinical liaison between patients, families, and the interdisciplinary hospice team, which may include physicians, social workers, chaplains, and volunteers. Additionally, they provide education and emotional support to families, help navigate complex medical decisions, and ensure compliance with regulatory standards, all while advocating for the patient’s wishes and enhancing quality of life during hospice care.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of practice
  • Minimum of 2 years of nursing experience, preferably in acute care, home health, or hospice
  • Strong assessment and clinical decision-making skills
  • Knowledge of end-of-life care, symptom management, and palliative care principles
  • Excellent communication and interpersonal skills for interacting with patients, families, and interdisciplinary teams
  • Ability to work independently and manage a caseload effectively
  • Basic computer proficiency for electronic medical records and documentation

Nice To Haves

  • Hospice or palliative care certification (CHPN or equivalent)
  • Experience in case management or care coordination
  • Familiarity with regulatory standards and compliance for hospice care
  • Experience providing patient and family education on end-of-life issues
  • Strong organizational skills with the ability to prioritize complex tasks
  • Compassionate and culturally sensitive approach to patient care
