Requirements

  • Graduate of an accredited school of professional nursing, BSN preferred.
  • Currently licensed as an RN through the Ohio State Board of Nursing.
  • Prior nursing experience in Hospice or Home Health setting preferred.
  • At least one year's medical/surgical nursing experience preferred.
  • BLS/CPR Certification required.
  • Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy.
  • Employee must provide proof of personal automobile liability insurance coverage upon request.
  • Independent decision-making skills and abilities to render nursing care in an independent setting.
  • Ability to rotate 'On Call' after hours, weekends, and holidays.

Nice To Haves

  • Experience with Electronic Medical Records (EMR) software is preferred.
  • Demonstrated ability to work autonomously as a registered nurse.
  • Effective communication skills (verbal and written).
  • Time management and organizational skills.
  • Excellent planning, organizing, and communicating.

Responsibilities

  • Performs home care visits after the initial assessment, collects ongoing data, and reports to the RN Clinical Supervisor, implementing the Plan of Care.
  • Provides skilled nursing care in the patient's home as ordered by the attending physician in conjunction with the Plan of Care.
  • Exercises autonomy within the position, using training, critical thinking, and knowledge of patients to make decisions.
  • Submits clear and appropriate documentation in a timely manner and documents patient education.
  • Communicates effectively to physicians, supervisors, and co-workers in a timely manner.
  • Promotes continuity of care with appropriate admissions, transfers, and charges.
  • Performs other duties as assigned.
