Requirements
- Current Registered Nurse (RN) license in Pennsylvania
- Home Health experience required
- Hands-on experience with OASIS documentation
- Minimum 1 year of acute care experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Ability to work independently and manage a flexible schedule
Responsibilities
- Deliver Personalized In-Home Care
- Perform comprehensive health assessments in patients’ home environments
- Develop and implement individualized care plans tailored to patient needs
- Monitor patient progress and adjust care plans to achieve optimal outcomes
- Provide Skilled Nursing Services
- Administer medications and treatments as prescribed
- Deliver wound care and other skilled interventions
- Ensure compliance with physician orders and care standards
- OASIS & Documentation Excellence
- Complete and document OASIS assessments accurately and thoroughly
- Maintain detailed, compliant clinical documentation
- Ensure adherence to state, federal, and agency regulations
- Collaborate & Educate
- Partner with physicians, therapists, and interdisciplinary team members
- Educate patients and families on disease management, prevention, and wellness
