RN / REGISTERED NURSE - HOME CARE CASE MANAGER
About The Position

Requirements
- Minimum of one (1) years recent experience as a professional nurse in a clinical setting.
- Essential: Experience In Hospital Or Acute Care Setting
- Clear Communication Skills Both Written And Verbal
- Able To Keep Confidential Information Regarding Patients, Team Members
- Able To Withstand Crisis Situations
- Has Skills To Provides Customer Service To Patients, Team Members And Visitors
- Proof Of Current/Valid Car Insurance
- Knowledge And Experience With Electronic Health Records
- Valid DL With No More Than 3 Moving Violations
- Essential: RN - Registered Nurse
- BLS - Basic Life Saving certification
- Essential: Graduate of an accredited RN program

- Home Health experience preferred.
Responsibilities
- Skilled Nursing Care and Comprehensive Assessment
- Teaching, Supervision, and Counseling Clients and Caregivers
- Interdisciplinary collaboration

- Monitors patient progress to ensure accomplishment of patient goals
- Oasis documentation
- Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)
