Healthy Beginnings Plus is an enhanced, comprehensive package of services developed by the Department of Human Services to assist low-income, pregnant women who are eligible for Medical Assistance to have a positive prenatal care experience. It includes, in addition to medical services, a variety of health promotion and counseling services, as well as home and community-based services. The Healthy Beginnings RN performs client assessment and triage including coordinating referrals to in-house clinical and community services, conducting family education, providing all aspects of nursing care, assisting physicians and physician extenders. Ensures that the client receives the mix of services appropriate to her needs in a timely fashion. Provides primary clinical support to the provider, patient and their family to assist in the diagnosis and treatment of the patient's condition. Follows up on diagnostic testing to ensure completion, accuracy, and filing of same in medical record to ensure continuity and quality of care. This role is full-time, 40 hours weekly; Days. A minimum of 2 years RN work experience is required.
