HOW YOU'LL WORK: You’ll provide oversight of all personnel and patient care services. You will be responsible for monitoring compliance, quality and safe delivery of services by following clinical processes. Post-acute care is at the center of improving health outcomes. We care for patients where they spend the majority of their time – in their homes. This privileged position allows us to see things that are invisible to a patient’s primary care or hospital physician, and to deliver the best possible care tailored to each patient’s setting. As the healthcare industry continues to evolve away from hospital-centric care, our work, caring for patients in their homes becomes more important than ever. Our success helping patients recover comes from engaging with them. We start at the facility bedside and coordinate each patient's transition back to their home, ensuring a smooth transition and that their care plan is right for them. We support complex, high-acuity, medically-at-risk aging patients. This is the most challenging group of individuals to serve, and we find it the most rewarding. And we do this all in partnership with health systems. Together we create a seamless care environment that enables patients to receive excellent care in the setting that best meets their needs.

Requirements

  • Current RN or PT license in the state of employment
  • Minimum of Three (3) years of experience as an RN or PT in an acute care setting
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands

Responsibilities

  • Ensure Start of Care compliance by our clinical team and focus on Outcome Monitors as assigned.
  • Participate in all liaison/administrative on-call rotation, and patient staffing as needed to manage after hour’s team, staffing needs and patient related issues.
  • Process complaint workflow reporting staffing trends, neglect, abuse or theft to Administrator for resolution & follow up
  • Complete patient visits as needed and assist with any patient issues.
  • Participate in managing cost of good working with monitoring supply costs, overtime, staffing, bonuses and visits per episode.
  • Select and onboard staff.
  • Collaborate with intake, providing daily direction, managing referral volume based on staffing availability.
  • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
  • Provide oversight for liaison and clinical staff and ensure timeliness of completion of work/charting as well as identify education needs for the team.
  • Manage On call, PTO, call offs & daily/weekly productivity review to ensure proper staffing coverage to meet the needs of the business in the field and internally.
  • Minimize Patient Risk: Ensure quality and safe delivery of home health services by our clinical team.
  • Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
