Midland Care Connection Midland Care Connection is a not for profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in Kansas. Community Care Hospice One of Midland Care Connection’s hospice programs, offered through its Community Care Hospice partnership, provides compassionate, personalized end of life care to individuals and their families in central and south central Kansas. As a not for profit organization with decades of experience, Midland Care focuses on comfort, dignity, and support for those facing life limiting illnesses, delivering services tailored to each person’s needs at home, or in partner facilities. Through the Community Care Hospice joint venture with local non profit retirement and senior living communities, the program extends this expertise across multiple counties, combining skilled clinical care with emotional and spiritual support to help patients and families navigate the final stages of life with compassion and respect. RN Case Manager In the Midland Care Connections Community Health hospice program, an RN Case Manager plays a critical role in coordinating and delivering high quality, patient centered end of life care. They are responsible for assessing patients’ medical needs, developing individualized care plans, and ensuring that patients receive appropriate symptom management, medications, and therapies to maintain comfort and dignity. The RN Case Manager also serves as the primary clinical liaison between patients, families, and the interdisciplinary hospice team, which may include physicians, social workers, chaplains, and volunteers. Additionally, they provide education and emotional support to families, help navigate complex medical decisions, and ensure compliance with regulatory standards, all while advocating for the patient’s wishes and enhancing quality of life during hospice care.
