About The Position

This is primarily an office-based position focused on processing hospice and out-patient palliative referrals. The role involves clinical data review and assisting providers in obtaining information to determine hospice eligibility. Responsibilities include triaging hospice RN calls, taking physician orders, coordinating pharmacy and DME needs, and assisting with remote patient monitoring (RPM). The position also requires participation in committees aimed at improving processes and assisting with scheduling. Additionally, the role involves orienting new staff, accurately reporting, recording, and documenting information, demonstrating knowledge of Medicare, Medicaid, and insurance regulations related to hospice, participating in clinical record reviews for quality and compliance, and maintaining and submitting logs, schedules, and reports within required time frames. Patient visits are necessary for troubleshooting RPM equipment and providing patient education.

Requirements

  • Two years of home care or hospice-related experience.
  • Experience in med-surg or acute care.
  • Ability to communicate effectively with all levels of the workforce, both clinical and non-clinical.
  • Understanding of hospice philosophy.
  • Knowledge of CMS regulations, compliance, documentation standards, and HIPAA.
  • Demonstrated computer skills.
  • Team-oriented with good organizational skills.
  • Ability to manage multiple tasks and meet deadlines.
