About The Position

Responsibilities include primarily office based position processing hospice and out-patient palliative referrals. Clinical data review and assisting the provider with obtaining information to determine hospice eligibility. The position will consist of triaging hospice RNs calls, taking physician orders, coordinating pharmacy & DME needs, assisting with RPM (remote patient monitoring) Will also participate in committees to improve processes and assists with scheduling. Assists with orientation of new staff, reports/records/documents information accurately, demonstrates knowledge of Medicare, Medicaid, and insurance regulations pertaining to hospice, participates in clinical record review for quality and compliance, maintains and submits logs, schedules and reports within required time frames. Patient visits are required for trouble shooting RPM equipment and patient education Position reports to FHH Associate Director and works directly with Clinical Services Manager (field) and Hospice House Unit Manager to assist with and coordinate daily duties in a fast paced hospice office setting to efficiently meet patients/caregiver needs.

Requirements

  • Two years of home care or hospice related experience, med-surg or acute care experience required
  • Must have ability to communicate effectively with all levels of the work force- clinical and non-clinical
  • Must have understanding of hospice philosophy and knowledge of CMS regulations, compliance, documentation standards and HIPAA
  • Must have demonstrated computer skills
  • Be team oriented
  • Have good organizational skills with the ability to manage multiple tasks and meet deadlines
