The Quality Improvement Specialist reports to the Director of Quality and Patient Experience and is an active member of the Quality and Performance Improvement Department. Responsibilities include abstraction of quality measures from electronic medical records systems using measure specifications, auditing clinical documentation based on expected best practices, and participation in improvement projects. This position will review patient charts for completeness and accuracy that reflects services provided, evidence-based practices, current patient status, individualized care plans, progress toward goals and outcomes of care. The Quality Improvement Specialist provides coaching and findings to the interdisciplinary team members as needed.

Requirements

  • Registered Nurse, currently licensed in Massachusetts in good standing.
  • Associate’s degree in nursing required; Bachelor’s degree in Nursing preferred.
  • Minimum three years of varied clinical experience as a professional nurse, and preferably one of the three years in a hospice, home care, or long term care setting.
  • Understanding of hospice philosophy and principles.
  • Experienced with Electronic Medical Records.
  • Proficiency in MS Office/Excel/PowerPoint/Outlook
  • Able to learn and use healthcare specific data entry systems
  • Ability to interpret regulatory standards for documentation according to industry standards.
  • Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation.
