PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community. This role is ideal for an experienced RN who enjoys collaboration, data-driven improvement, and making a meaningful impact across an organization. What You’ll Do Under the direction of the Associate Director of Quality Improvement, you will support organization-wide quality assurance and performance improvement initiatives while serving as a key leader in infection control and regulatory readiness.
