Responsibilities Responsible to deliver results improvement with the Stroke Program with multiple stakeholders across several departments, service lines, sites and/or Southcoast corporate entities through direct support and collaboration. Recommend actions for workflow process changes to hardwire results supported by data-driven analyses from variances related to patient outcomes and care. Coordinate, maintain accreditation activities related to stoke certification and support/prepare for regulatory-type audits, surveys, or grant work. Create, initiate with a multi-disciplinary team, support groups for stroke survivors and families, by offering a range of resources to achieve a desired long-term outcome with needed care plans and emotional support. Participate with educational efforts with healthcare professionals, EMS outreach, community related to evidence-based standards of stroke care. Review, abstract chart data and complete data entries as needed proactively seek work or assignments as workload permits. Acquire and employ knowledge for reporting, analysis and data use from various databases. Work cooperatively with department colleagues.
