We are seeking an experienced and forward-thinking RN Program Manager for Erie and Niagara County to design, launch, and oversee our new Home Infusion Therapy Service Line. This is a senior clinical leadership role for a nurse who combines hands-on infusion expertise with strong program-development skills. If you’re passionate about advancing infusion care in the home setting—and ready to build a best-in-class service line from the ground up—this role is for you. Hours: Monday: Friday 8:30a-5:00pm Location: Office and Community Sign On Bonus: $10,000 for full-time work paid quarterly: After 90 days: $2,500 After 6 months: $2,500 After 9 months: $2,500 After one (1) year: $2,500

Requirements

  • NYS RN license
  • Minimum 5 years clinical nursing experience with strong infusion background
  • Experience in home infusion, specialty pharmacy, acute/ambulatory infusion, or similar
  • Program development or clinical leadership experience

Nice To Haves

  • CRNI® or IgCN® (strongly preferred)
  • INS training or other advanced infusion certifications (a plus)

Responsibilities

  • Develop clinical models, workflows, and operational systems for all home infusion services
  • Create policies, protocols, and procedures aligned with NYS and national infusion standards
  • Establish referral, intake, eligibility, and pharmacy coordination processes
  • Partner with leadership on program design, capacity planning, and growth
  • Serve as the agency’s infusion therapy clinical expert
  • Provide oversight, mentorship, and competency validation for infusion RNs
  • Ensure safe delivery of IVIG, biologics, antibiotics, hydration, and other ordered therapies
  • Oversee assessments, care plans, documentation, and clinical monitoring
  • Set escalation pathways and emergency procedures specific to infusion care
  • Ensure full compliance with NYSDOH, LHCSA, CMS (as applicable), and agency policies
  • Lead quality initiatives, chart reviews, incident investigations, and performance monitoring
  • Track key infusion metrics such as adverse events, adherence, and documentation accuracy
  • Support audits, surveys, and program-readiness activities
  • Recruit and onboard qualified infusion RNs
  • Develop and deliver infusion-specific training and competency programs
  • Maintain high clinical standards and provide coaching and performance support
  • Work closely with prescribing providers, infusion pharmacies, and referral sources
  • Educate patients and caregivers on infusion treatment, safety, and expectations
  • Serve as a clinical liaison across internal and external partners
