The position ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components. The essential functions include developing, implementing, and enhancing special projects that promote inpatient/outpatient continuity of care and parent engagement. The role ensures the provision of high quality, individualized, comprehensive, cost-effective patient care across the continuum. Responsibilities also include initiating, tracking, analyzing, and managing all program quality improvement and research projects, as well as coordinating functions with interdisciplinary medical care and liaising with patients, families, medical staff, and referring physicians. Additionally, the position involves developing educational materials and programs for patients, nursing and ancillary staff, and physicians, as well as clinical protocols, policies, and procedures including community education and public relations.

  • Bachelor's Degree, required.
  • Registered nurse in the State of Ohio, required.
  • Three years of registered nurse experience, required.
  • Masters Degree in Nursing, preferred.
