Job Description The Triage and Assessment Counselor/Nurse provides telephonic and in-person risk assessment and crisis intervention, initial contact with referral sources and potential patients, determines appropriate level of care and maintains communication with referral sources, families, patients and unit staff. Triage and Assessment Counselor/Nurse receives individual/family walk-ins with compassion, empathy, a sense of urgency, and to ensure each person receives an appropriate assessment. Thorough and accurate documentation is a critical component of the job. Requirements

  • To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description.
  • The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position.
  • Staff must demonstrate knowledge and skill in provision of services to varied population of patients in an inpatient and partial hospital setting.
  • Education/Licensure: Licensed Physicians, APRN’s, Licensed Psychologist, Licensed Social Workers, Licensed Professional Counselors, Registered Nurses, Masters in Social Work, Masters in Counseling, Masters in Marriage and Family Therapy, or Masters of Psychology required.
  • Experience: Prior emergency department, intake, assessment, admission and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting preferred, but not required.
  • Licensure/Certification: If Registered Nurse, licensure and registration by the Tennessee Board of Nurse Examiners as a registered nurse.
  • Knowledge: Prefer knowledge of crisis intervention and risk assessment, psychiatric diagnoses symptomology, basic chemical dependency, age specific growth and development and crisis and behavior management.
