About The Position

JOB SUMMARY: Provide exceptional in-home IV infusion therapy, nursing care, and response management. In addition, the Home Infusion Registered Nurse will administer and monitor patients in the home during treatment and provide the necessary education on treatment regimen, condition management, and treatment self-administration when appropriate.? GENERAL REQUIREMENTS : · Performs all job responsibilities in alignment with the mission and vision of the organization. · Performs other duties as required and completes all job functions as per departmental policies and procedures. · Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs). · Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time. · Wears protective clothing and equipment as appropriate. GENERAL SKILLS: · Ability to communicate in English, both verbally and in writing. · Additional languages preferred. · Strong written and verbal skills. · Basic Computer Skills WORKING CONDITIONS: · General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels. · May be exposed to high noise levels and bright lights. · May be exposed to limited hazardous substances or body fluids, or infectious organisms. · May be required to change from one task to another or different nature without loss of efficiency or composure. · Periods of high stress and fluctuating workloads may occur. · May be scheduled as needed including overtime. PHYSICAL REQUIRMENTS & DEMANDS: · Have near normal hearing: Hear alarms/telephone/normal speaking voice. · Have near normal vision: Clarity of vision (both near and far), ability to distinguish colors. · Have good manual dexterity. · Have good eye-hand foot coordination. · Ability to perform repetitive tasks/motion. · Continuously within shift (67-100%): Standing, Walking. · Frequently within shift (34-66%): Bending/Stooping, Pushing/Pulling, Lift/carry up to 20lbs, Lift/carry greater than 20 lbs. with assistance. · Occasionally within shift (1-33%): Sitting, Climbing, Twist at waist, Lift/Carry greater than 50 lbs. with assistance, Reaching above shoulder. JOB SPECIFIC COMPETENCIES: · Care for the individual based on personalized Plan of Care · Informs the Pharmacy or Nursing manager and any other appropriate personnel of changes in the client’s conditions or drug management issues and needs. · Counsel's client and family through care process while updating family on a regular basis. · Assures proper maintenance of clinical records in compliance with local, state, and federal laws. · Participates in home infusion performance improvement activities and continuously staying informed about changes in the field of nursing and home health care. · Provide a broad range of infusion therapies in our patients' homes. · Make a difference by using your clinical IV experience and personal touch to assist our patients recover in the comfort of their own home. · Provide one-on-one home infusion nursing care visits and patient education in accordance with the physician's plan of treatment. · Train patients in the self-administration of treatments and how to quickly recognize and respond to symptoms of adverse physical reactions to treatments. · Use excellent written and verbal customer service skills in order to interact with the patient, physician, pharmacist and other partners. · Document all interactions and communication with patients, plan of treatment, assessment, treatments, patient responses/reactions to treatment, status of disease process, and training activities in a timely manner. · Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. · Participate in staff development meeting. · Continually strive to improve his/her nursing care skills by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations, and individual research and reading. · Participate in the development, and periodic revision, of the physician's Plan of Treatment and processes change orders, as needed. · Maintain an on-going knowledge of current drug therapy. · Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations. · Perform on-call responsibilities and provide on-call service to assigned patients/families when needed.
