About The Position

The PRN RN - Kids Path position at AuthoraCare Collective involves managing a case load for various services within the Kids Path program, which may include hospice, home health, and perinatal services. The role requires collaboration with an interdisciplinary team to assess patient needs, establish care goals, and implement interventions. The RN will also work closely with physicians to develop medical plans and provide emotional support to patients and caregivers.

Requirements

• Graduate of an accredited School of Nursing.
• Currently licensed as an RN in North Carolina and in good standing with NCBON.
• At least one year of experience in an acute or home setting (two years preferred).
• At least one year of pediatric experience required.
• Bachelor's degree in nursing preferred.
• Familiarity with Medicaid, CAP, Case Management, and Home health is a plus.

Responsibilities

• Manage RN case load for all/multiple services of the Kids Path program.
• Assess and identify patient/caregiver complications.
• Establish patient/caregiver care goals and implement interventions.
• Collaborate with physicians to establish and implement medical plans of care.
• Maintain therapeutic relationships with patients and caregivers, providing emotional support and teaching.
• Communicate effectively with team members, patients, caregivers, and other healthcare professionals.
• Travel to and function in the homes of patients/caregivers within the service area.
