About The Position

As our RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management. Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs. To be successful in this role you must be knowledgeable in community resources and alternate care facilities.

Requirements

• High School Graduate Education level necessary to obtain required licensure and certifications.
• 1-2 years of previous experience.
• Registered Nurse: CA required
• Basic Life Support - AHA required
• Advanced Cardiac Life Support AHA required

Nice To Haves

• Bachelors Of Nursing preferred

Responsibilities

• PATIENT ASSESSMENT/REASSESSMENT
• Perform patient assessment/reassessment, including giving or receiving comprehensive change of shift and intermittent report, obtaining the nursing history, conducting a physical assessment, establishing priorities for care.
• Assess patient's learning needs, readiness for discharge and ability of the patient/family/SO to manage illness after discharge.
• Completes both computer and hand written documentation using correct format in an accurate, precise, timely, legible and complete manner .
• Provide direct patient care, as required. Assist physician with procedures, as necessary. Is competent in IV Therapy.
• Report changes in patient's condition to physician.
