Become a part of our caring community and help us put health first The Registered Nurse Post‑Acute Care Coordinator assesses and evaluates members' needs to achieve and maintain wellness by guiding members and their families and facilitating access to resources that support appropriate care and overall wellbeing. You will employ a variety of strategies, and techniques to manage members' physical, environmental, and psychosocial health issues. As the Registered Nurse Post‑Acute Care Coordinator, you will identify and address barriers to reduce the risk of hospitalization following emergency department coordination and ongoing patient support, while collaborating with branch case managers and leadership. Work assignments are varied and frequently require interpretation and determination of courses of action. The role requires an understanding of departmental, segment, and organizational strategies and operating goals, including escalation pathways and clinical best‑practice guidelines. You will follow established procedures, exercises autonomy in work methods, and operate with minimal supervision while seeking guidance. You will report to the Quality Assurance Manager.
