About The Position

This is a on call/per diem position working on weekdays, day shift and traveling to see patients. The RN provides care management, care coordination, skilled care and services for at-risk frail elder patients with complex medical and psychosocial issues. Population-based disease management approaches and application of clinical protocols are integral to the program. This position is a core member of the patient centered PACE interdisciplinary team (IDT). The goals of care management are to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality. This position assists providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient's functional status and independence. This position is responsible for the planning, overseeing and monitoring of all nursing and unlicensed caregiver services to each patient on their panel within the context of an IDT. This position requires occasional local travel. Requirements

  • Coursework/Training Relevant training applicable to the position, as specified by the Nurse Manager.
  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association
  • Relevant certifications applicable to the position, as specified by the Nurse Manager

Nice To Haves

  • Bachelor's Degree Nursing.
  • 1 year Home health RN experience.
