About the position

The Registered Nurse (RN) position involves providing comprehensive nursing care to a designated group of residents, utilizing the nursing process which includes assessment, diagnosis, planning, implementation, and evaluation. The RN collaborates with physicians and other healthcare team members to ensure effective treatment and care coordination, while also exercising leadership skills to direct and delegate tasks to the care team, including LPNs, CNAs, and Unit Secretaries.

Responsibilities

  • Assess the patient's physiologic health status through interviews and chart reviews.
  • Evaluate gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
  • Determine mobility, sensory deficits, and skin condition, adjusting care plans as necessary.
  • Assess pain levels and manage pain effectively.
  • Communicate and document patient's health status and care plans accurately.
  • Assess patient's psychosocial health status and identify cultural/ethnic requirements.
  • Develop criteria for measuring patient outcomes and identify potential patient problems.
  • Formulate and document individualized care plans, prioritizing care activities.
  • Implement the plan of care while ensuring patient safety and comfort.
  • Administer prescribed treatments and medications accurately and safely.
  • Evaluate the effectiveness of care provided and document outcomes.
  • Collaborate with care team members and provide direction to support staff.
  • Perform skin care procedures and preventive measures for decubitus ulcers.
  • Educate patients and families about conditions and care plans.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • BCLS certification; ACLS certification preferred.
  • Bachelor's Degree in Nursing preferred, or enrollment in an accredited program within 24 months of employment.
