This position is hybrid in Arizona working Monday-Friday, 40 hours/week. POSITION SUMMARY This position evaluates, identifies, plans, coordinates and leads enterprise and facility-specific patient safety activities in collaboration with enterprise and facility leaders. This position actively communicates with identified internal and external customers to reduce and prevent patient harm.

Requirements

  • Requires Bachelor’s degree or equivalent in nursing or other healthcare related field and an RN license in state worked.
  • Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. Additional certification or continuing education may be required based on area of practice.
  • Requires a proficiency level typically attained with five years acute care clinical experience.
  • Requires ability to perform complex statistical analysis and highly developed problem solving skills.
  • Requires the ability to manage programs and projects.
  • Requires demonstrated excellence in interpersonal and written communication skills.

Nice To Haves

  • Certified Professional in Healthcare Quality (CPHQ), Certified Professional in Patient Safety (CPPS) is preferred.
  • Possesses experience with process improvement concepts and tools, group facilitation, data management, and analysis including graphic development and presentations is highly desirable.
  • Knowledge of regulatory/accreditation requirements is desirable.
  • Additional related education and/or experience preferred.

Responsibilities

  • Assists in the reinforcement and consistent use of a standardized patient safety taxonomy and evaluation system. Serves as a consultant to system and facility leadership, as well as executive sponsors.
  • Actively identifies patient safety opportunities through concurrent event triage and review. Coordinates with leaders to investigate and respond to quality/patient safety concerns, events, significant care issues and trends. Conducts surveillance, analysis and data reporting for patient safety, while reinforcing regulatory/accreditation requirements.
  • Participates in implementation of standardized patient safety clinical practices at the enterprise and facility levels. Develops and analyzes data for administrative and clinical decision-making to support safe patient care practices and team member safety.
  • Determines risk for exposure of harm/potential harm to patients in response to identified opportunities, outcomes, etc., using a standardized taxonomy and evaluation process. Assists in the determination of the level of containment/control required for: immediate harm, risk for harm in current process and future potential for harm in partnership with enterprise and facility leaders.
  • Facilitates root cause and/or apparent cause analysis activities, variation reduction and supports identification of process improvements related to patient safety using standard improvement methodologies and tools. Collaborates with and provides direction for leaders for patient safety event investigation, closure and action planning.
  • Leverages lessons learned and communicates recommendations/required actions to avoid future patient harm. Presents data and trends, communicates identified deficiencies surfaced through internal surveillance and monitoring and conveys the status of implementation to key stakeholders and identified facility and enterprise committees and leadership teams.
  • Serves as a subject matter expert on internal and external patient safety matters. Collaborates with enterprise and facility teams: care management teams, clinical consensus groups, enterprise and facility leadership, risk management and other groups as appropriate. Provides education and resources for safety taxonomy, enterprise safety data collection systems, assists and facilitates identification of deviations in generally accepted performance standards (GAPS) and consults with leaders and team members on patient safety standards.
  • This position has enterprise-wide responsibility, as well as facility-specific affiliation. This position functions autonomously and provides ongoing patient safety communication to internal and external customers. Internal customers include but are not limited to: enterprise and/or facility leaders, physicians and Banner Health team members. External customers include but are not limited to: insurers, accreditation/regulatory bodies, patients, families and the community.
