About The Position

The RN, Patient Navigator manages and coordinates aspects of care for treatment of three multi-disciplinary clinics (Aero digestive Clinic, Cleft and Craniofacial Clinic and Pediatric Plastic Surgery Clinic). The Navigator offers medical guidance, acts as support and guides patients and their families. The Navigator demonstrates knowledge skill and competency when applying evidences based practices. The Navigator will be responsible for developing relationships with all multi-disciplinary providers and the primary care providers to assist in education, creating increased access to care. The Navigator serves as a patient advocate, liaison and advisor/educator in effort to assist the patient and family. The Navigator must be knowledgeable of current method of treatment, new pharmaceutical agents, available community resources, insurance, financial assistance, counseling and other support services. The RN, Patient Navigator is responsible for ensuring coordinated quality, seamless access to care. The Navigator manages and coordinates all aspects of care for treatment with the patient population within the pediatric multi-disciplinary clinics. The Navigator offers medical guidance, supports and guides patients and their families. The Navigator demonstrates knowledge skill and competency when applying evidences based practices. The Navigator will be responsible for developing relationships with all multi-disciplinary providers and the primary care providers to assist in education, creating increased access to care The Navigator serves as a patient advocate, liaison and advisor/educator in effort to assist the patient and family in navigating the continuum of care. The Navigator must be knowledgeable of current method of treatment, new pharmaceutical agents, available community resources, insurance, financial assistance, counseling and other support services. The RN, Patient Navigator serves as the single point of contact for patients who are referred into the Aero digestive Clinic, Cleft and Craniofacial Clinic and the Pediatric Plastic Surgery Clinic in effort to facilitate the most appropriate entry point for treatment to ensure the appropriate plan of care is implemented based on the patient's chief complaint and clinical condition. The Navigator serves as a liaison between the patient and family and the patient's network of care provider(s). The Navigator is responsible for educating the patient and family on diagnosis, treatment options, insurance options, scheduling appointments, completing paperwork, and securing the referral into the pediatric multi-disciplinary clinics. The Navigator serves as a patient advocate by removing barriers patients and their families may encounter during the treatment process. The Navigator is responsible for connecting the patient and family with community resources that may help them stay on track with treatment or improve their quality of life. He/She will assist, as deemed appropriate, in research activities. He/She will work with the leadership team to collect data, track outcomes and support the strategic planning processes. He/She will develop and deliver quarterly reports on identified outcome metrics attained through performance improvement activities. He/She will work to ensure compliance standards. He/She will deliver compassionate, holistic care, while supporting the rights and dignity of all patients. The Navigator is responsible for clinical oversight of the Aero digestive Clinic, Cleft and Craniofacial Clinic, and the Pediatric Plastic Surgery Clinic. This includes supervision of all clinical team members within the multi-disciplinary office space in the Massoud Building. This will include but is not limited to interviewing new applicants, training new employees, clinical check off, hiring, any disciplinary actions, and firing of employees with manager authorization. The Navigator demonstrates knowledge, skill and competency when applying evidence based practices, theory, principles and processes. He/She will have experience working collaboratively with medical teams and has the ability to build and maintain solid interdepartmental relationships. The Navigator will ensure that all personal, patient and departmental safety guidelines are upheld.

Requirements

  • Graduate from an accredited School of Nursing
  • Greater than 2 years experience in related field
  • Current licensure to practice nursing in the state of Tennessee
  • BLS completed prior to going to the unit/ department and maintained going forward
  • Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.
