At the Mass General Brigham Heart & Vascular Institute, our cardiology, cardiac surgery, and vascular care teams collaborate seamlessly to provide comprehensive, patient-centered heart and vascular care. Our cardiologists focus on diagnosing and managing heart conditions, our cardiac surgeons perform life-saving procedures like bypass surgery and valve repair, and our vascular specialists treat circulatory disorders affecting the arteries and veins, ensuring each patient receives a personalized, multidisciplinary approach for optimal heart and vascular health. The Patient Navigator is a Registered Nurse who will function in a clinical integration and patient care coordination role in partnership with vascular medicine and vascular & endovascular surgery clinicians and other key stakeholders. The role is responsible for directing the patient’s progression from referral for consultation through ambulatory visits, testing, and procedures in collaboration with the intradisciplinary care teams – including physicians, advanced practitioners, nursing, technologists, and non-clinical support staff. The primary focus of the role is identification of patients who may be candidates for vascular disease care programs and facilitation of patient triage into the appropriate pathway. The role may also be asked to assist with clinical care in addition to patient triage to support the complex care needs of patients, their families, and caregivers. Requirements

  • Associate's Degree Nursing required or Bachelor's Degree Nursing preferred
  • Registered Nurse [RN - State License] - Generic - HR Only required
  • Basic Life Support [BLS Certification] - Data Conversion - Various Issuers preferred
  • Clinical nursing experience 0-1 year required
  • Proficient knowledge of nursing principles, practices, and procedures.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced and dynamic healthcare environment.
  • Demonstrated commitment to continuing education and professional development.
  • Familiarity with electronic medical records (EMRs) and healthcare technologies.

Nice To Haves

  • Ambulatory care or outpatient experience 1-2 years preferred

Responsibilities

  • Deliver direct patient care and support in our ambulatory care unit, ensuring high nursing standards.
  • Conduct patient assessments, devise care plans, and assist on outpatient procedures with a focus on patient comfort and safety.
  • Educate patients and families on treatment plans, medications, and post-procedure care.
  • Collaborate closely with healthcare professionals to coordinate comprehensive patient care.
  • Administer medications, perform IV therapy, and maintain accurate patient records in compliance with regulations.
  • Uphold infection control protocols and maintain a safe environment for patients and staff.
