It involves assisting patients in understanding their diagnosis, treatment options, and the resources available, while also reviewing medical documentation to assist physicians in obtaining complete and accurate documentation of their medical treatment plan. The role serves as an essential link between patients and all other care providers, facilitating appointments for consults and support services within established service standards. The position requires following patients throughout the care continuum, collaborating with inpatient care management resources, and demonstrating the knowledge, skill, and coordination to provide nursing care and guidance from screening to survivorship. Additionally, the role involves educating patients and their families, navigating treatment-related problems, and supporting patients during difficult decision-making periods.
