About The Position

The RN, Patient Navigator manages and coordinates aspects of care for treatment of three multi-disciplinary clinics (Aero digestive Clinic, Cleft and Craniofacial Clinic and Pediatric Plastic Surgery Clinic). The Navigator offers medical guidance, acts as support and guides patients and their families. The Navigator demonstrates knowledge skill and competency when applying evidences based practices. The Navigator will be responsible for developing relationships with all multi-disciplinary providers and the primary care providers to assist in education, creating increased access to care. The Navigator serves as a patient advocate, liaison and advisor/educator in effort to assist the patient and family. The Navigator must be knowledgeable of current method of treatment, new pharmaceutical agents, available community resources, insurance, financial assistance, counseling and other support services.

Requirements

  • Graduate from an accredited School of Nursing.
  • Current licensure to practice nursing in the state of Tennessee.
  • BLS completed prior to going to the unit/department and maintained going forward.
  • Greater than 2 years of experience in related field.

  • Outpatient referrals patterns experience.
  • Pediatric experience.

Responsibilities

  • Educate patients and families on diagnosis, treatment options, and insurance options.
  • Secure referrals into pediatric multi-disciplinary clinics.
  • Connect patients and families with community resources.
  • Assist in research activities and collect data for performance improvement.
  • Ensure compliance with standards and deliver compassionate care.
  • Supervise clinical team members and manage hiring and training processes.
