About The Position

The RN – Patient Assessment Team specializes in comprehensive participant evaluations to support individualized care planning. This role conducts initial and ongoing assessments, collaborates with the interdisciplinary team, and ensures accurate documentation for regulatory compliance. Essential Functions and Work Responsibilities Perform detailed physical, functional, and psychosocial assessments for new and existing participants. Develop and update individualized nursing care plans based on assessment findings. Collaborate with providers and interdisciplinary team members to formulate Plans of Care. Document all assessment data and maintain accurate participant records. Provide health education and counseling to participants and families. Identify changes in participant condition and communicate promptly to providers. Participate in family meetings and interdisciplinary team discussions. Ensure compliance with all regulatory and InnovAge standards. Current First Aid and BLS certification (American Heart Association or Red Cross). 1+ year experience with frail or elderly populations. PREFERRED BSN degree. 1+ year acute care experience. 3+ years healthcare experience with geriatrics emphasis.
