This role will primarily conduct home visits to PACE participants in the Everett area. As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The RN's responsibilities including nursing assessments, care planning, interventions, medication administration/management, treatments, nurse delegation and patient teaching. The Registered Nurse utilizes nursing judgment and documents findings in accordance with agency policies. Performs nursing duties primarily in the community but may be called upon to deliver services in other care settings including the Center and Clinic. Under the direction of the primary care physician, provides oversight to licensed practical nurses, nursing aides and nursing students who are implementing the plan of care.
