About The Position

The RN, Care Coordinator at Hope Healthcare is responsible for assessing and identifying the needs of patients and their families, coordinating care plans with the Interdisciplinary Group (IDG), and providing palliative and supportive care. This role emphasizes outstanding patient care and customer service, ensuring that the needs of terminally ill patients are met with compassion and professionalism.

Requirements

  • Current RN license in the state of Florida.
  • Minimum one year of in-patient nursing or hospice experience.
  • Experience with EMR/EHR systems.
  • Valid driver's license and automobile insurance.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Ability to manage emotional stress associated with caring for terminally ill patients.
  • Physical ability to access patient rooms and perform essential functions of the job.

Nice To Haves

  • Experience in hospice or home health care settings.
  • BLS certification from the American Heart Association or Red Cross.

Responsibilities

  • Assess and identify patient/family needs using the nursing process.
  • Coordinate the Plan of Care with the Interdisciplinary Group (IDG).
  • Provide palliative and supportive care to patients and families.
  • Document patient/family contact information in the EMR and communicate with the IDG.
  • Complete clinical admissions for all Company services, explaining services to potential patients/families.
  • Educate patients/families on admission requirements and address immediate needs.
  • Initiate skilled nursing interventions to prevent complications and maximize comfort.
  • Obtain Physician orders and complete documentation per Company policy.
  • Act as the Company representative at assigned facilities, facilitating referrals to all service lines.
  • Communicate frequently with Admissions to ensure timely follow-up and prepare for next assignments.
  • Provide care for GIP patients in a hospital, including daily visits and documentation.
