About the position
Comfort Home Care is seeking full-time Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide compassionate home care for mental health patients. This role involves making meaningful home visits, conducting assessments, and coordinating care to enhance the well-being of patients. The position offers flexible work-from-home options and competitive pay, making it ideal for both new graduates and experienced nurses.
Responsibilities
- Provide thorough initial and ongoing assessments to identify each client's unique medical needs and level of functioning.
- Initiate and adapt the plan of care as necessary, performing OASIS assessments when needed.
- Coordinate services and inform physicians and colleagues of any changes in the client's condition.
- Collaborate with patients, families, and other healthcare providers in planning care and discharge.
- Prepare clinical notes and submit required paperwork punctually, in line with agency policies.
Requirements
- Valid RN or LPN license.
- Experience in home care or community care is preferred but not mandatory.
- Ability to perform assessments and coordinate care effectively.
