Seeking a Registered Nurse or Licensed Practical Nurse for Afternoons and Evenings from 2pm-8pm Soar with team Phoenix! Spousal Insurance PTO Orientation and training tailored to your needs as a new hire Motivational PHC culture, training, and Supportive Home Health Team We are Medicare Certified and State Licensed Responsibilities Skilled Visits Day-to-day patient interaction Education on Health Conditions, such as CHF, COPD, Diabetes Wound Care Medication Management Changes dressings, bandages and contraptions as prescribed Documentation of patient’s condition and pointing out significant changes, to be reported to the physician. Working alongside other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist. Requirements

• Home health experience required
• Missouri or Compact RN or LPN License
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license

Responsibilities

• Skilled Visits
• Day-to-day patient interaction
• Education on Health Conditions, such as CHF, COPD, Diabetes
• Wound Care
• Medication Management
• Changes dressings, bandages and contraptions as prescribed
