About The Position

The RN - Operations - Home Health & Hospice provides quality & safe delivery of home health & hospice care to assigned patients. This RN participates in interdisciplinary team conferences to ensure that the patient’s physical and emotional care is met with emphasis on palliation of symptoms and developing a patient centered POC.

Requirements

• Graduate of an accredited school of nursing.
• Two years of experience as a Registered Nurse or one year of experience in a Medicare Certified Home Health or Hospice Agency.
• Current Registered Nurse in the State of Michigan.
• Able to obtain Wisconsin license within 60 days of hire.
• BLS required within 30 days of hire.
• Must possess valid driver's license and reliable vehicle.

Responsibilities

• Provide quality and safe delivery of home health and hospice care to assigned patients.
• Participate in interdisciplinary team conferences.
• Ensure that the patient’s physical and emotional care is met.
• Emphasize palliation of symptoms.
• Develop a patient-centered plan of care (POC).

Benefits

• Level 1 benefit package for positions listed as 30 hours per week or greater.
• Level 2 benefit package for positions listed as 20 – 29 hours per week.
• Non-benefited for positions listed as less than 19 hours per week.
